Family Health Services Receives $256,000 Grant from Intermountain Community Care Foundation to Address Social Drivers of Health in Burley and Rupert

BURLEY, Idaho — Family Health Services (FHS) is proud to announce the receipt of a generous $256,000 grant from Intermountain Community Care Foundation to strengthen support for patients in the Burley and Rupert communities whose health is affected by social drivers of health, including housing instability and difficulty paying essential utility costs.

The three-year grant will enable FHS to expand efforts to identify and address non-medical factors that significantly influence health outcomes. Through clinical screenings, patients experiencing social needs will be connected with resources and services designed to improve stability, health, and overall well-being.

Funding will support outreach, education, and resource navigation services focused on helping patients access emergency housing and utility assistance. The initiative will prioritize individuals and families who are at risk of homelessness or are currently experiencing housing or utility instability, helping them remain safely housed while maintaining access to critical healthcare services.

The grant will also support FHS in strengthening and leveraging community partnerships throughout the Burley and Rupert area, creating a more coordinated network of support services for patients. By working closely with local agencies, nonprofits, and community organizations, FHS aims to build sustainable solutions that address the root causes of health disparities and improve quality of life for residents.
