The Family Health Services Assertive Community Treatment (ACT) Program provides comprehensive, individualized mental health services for adults with serious mental illness. Our multidisciplinary team meets people where they are—literally and figuratively—delivering care in homes, workplaces, and communities. FHS ACT program serves adults in the IDHW Region 5 which includes the counties of Blaine, Camas, Cassia, Gooding, Jerome, Lincoln, Minidoka and Twin Falls.

About the ACT Program

What is Asserive Community Treatment (ACT)?

Assertive Community Treatment is an evidence‑based, multidisciplinary team approach to mental health treatment designed for individuals who need a higher level of support than traditional outpatient services can provide. ACT services are delivered in the community rather than an office setting and focus on long‑term recovery, stability, and quality of life.

At Family Health Services, our ACT program is built on dignity, respect, and partnership. We work collaboratively with participants, families, and community partners to reduce hospitalizations, improve daily functioning, and support meaningful life goals.

Who We Serve

The ACT Program serves adults with serious and persistent mental illness who may:

  • Experience frequent psychiatric hospitalizations or emergency room visits
  • Have difficulty engaging in traditional outpatient treatment
  • Need intensive support to live independently in the community
  • Experience co‑occurring mental health and substance use challenges

Participation is voluntary and based on clinical appropriateness and eligibility criteria.

Our Multidisciplinary Team

ACT services are delivered by a dedicated, integrated team, which may include:

  • Psychiatrists and psychiatric nurse practitioners
  • Licensed clinical social workers and therapists
  • Nurses
  • Case managers
  • Community-Based Rehabilitation Specialists
  • Peer support specialists
  • Substance use specialists

This team works collaboratively to ensure continuity of care and rapid response to changing needs.
